Misplacement of temporary pacing wire into the left ventricle via an anomalous vein.

چکیده
We report on a case where a temporary pacing wire that was inserted through the left internal jugular vein was subsequently noted to follow an unusual course on the chest radiograph. A 12-lead electrocardiogram showed right bundle branch block pattern during pacing, suggesting that the lead was pacing the left ventricle. Computed tomography of the chest demonstrated that the likely path of the pacing wire was through an anomalous vein that originated from the left subclavian vein and drained into the left atrium. The importance of paying careful attention to the chest radiograph and electrocardiogram after transvenous pacing are discussed.

Treatment of a malpositioned transcutaneous ventricular pacing lead in the left ventricle via direct aortic puncture.
An 81-year-old woman with progressive cough was hospitalized 2 weeks following transcutaneous pacemaker implantation. Imaging revealed an absent brachiocephalic vein and aberrant course of a ventricular lead into the aorta with implantation into the left ventricle. We describe the unusual anatomic course, diagnosis, and surgical extraction of a malpositioned pacer lead.
متن کاملTemporary epicardial pacing wire removal: is it an innocuous procedure?
The safety and efficacy of temporary pericardial pacing wires have been accepted and their use is common after cardiac operations. Complications related to pacing wire removal are unusual but it can be serious and even catastrophic. We report an unusual case of bleeding due the laceration and rent created in the saphenous vein graft wall by the metallic tip of the pacing wire at the time of pac...
متن کاملCoronary sinus dissection during left ventricular pacing electrode implantation.
Coronary sinus (CS) dissection during biventricular pacing electrode implantation is a complication that rarely develops. A 71-year-old female with recurrent ventricular tachycardia, heart decompensation, and poor left ventricular function because of dilated cardiomyopathy was admitted for the implantation of a cardioverter-defibrillator for biventricular pacing. During the operation, we experi...
متن کاملPacing and defibrillation lead exchange without vein puncture.
BACKGROUND During lead implantation, venous access is generally achieved by puncturing the subclavian or axillary vein. Sometimes, although rarely, after lead positioning, the lead must be changed because of its inadequate mechanical stability or poor pacing parameters. This report concerns a technique of lead exchange that avoids an additional vein puncture. METHOD The tip of the lead, that ...
متن کاملTransvenous pacemaker electrodes placed unintentionally in the left ventricle: three cases.
Three patients are described in whom pacemaker electrodes were unintentionally placed within the left ventricle, followed by considerable delay before the error was recognized. In two cases temporary pacemaker wires were inserted into the subclavian artery and passed along a retrograde course. One patient required urgent surgery for acute arterial obstruction on removal of the wire. In the thir...
